This is 100% the custom house to use if you're looking for something special and one of one.

All the connoisseurs and celebrities use them.

Another option is Bamford. They do very good work. Harder to come by these days but exciting

Generally speaking those are my favorite custom houses that offer truly one-off pieces that stand apart in a crowded market...and don't compromise on quality
